Crime comedy follows a relationship between a psychiatrist and emotionally-insecure mob boss.

Successful psychiatrist Ben Sobol, encumbered by difficulties with his family and a lack of challenge from his steady flow of clients, inadvertently takes mafia don and self-styled "godfather" Paul Vitti on as a patient after crashing into one of his cars one fateful day. Vitti, a once-fearsome criminal now suffering from panic attacks and an inability to bump off his enemies, turns to Sobol to help him muscle through his problems, seeking to move past his inner turmoil in time for a gathering of dons from various cities just as the FBI compels the good doctor to break confidentiality norms by spying on his unlikely patient.
